Waiver Agreement

I hereby authorize the directors of the Hardwood Palace Summer Basketball Camp to act for me according to their best judgment in any emergency requiring medical attention. I release the camp personnel and employees of Hardwood Palace from all liability for any injury and illness incurred while at basketball camp. I have insurance covering the Camper in case of an injury or illness.
